School
nurses are responsible for first aid care of children who are injured
or become ill while under school supervision. In such an emergency,
every effort is made to contact the family. The success of this
procedure is dependent upon accurate information from parents. An
emergency information card/form is sent home in September from each
school and should be returned immediately to the appropriate school
office, classroom teacher, or homeroom teacher. If your contact information changes during the school year, please notify the school as soon as possible.
Immunizations: New
York State requires all children entering school to be immunized
against measles (both rubella and rubeola), polio, diptheria, and mumps.
The Hepatitis vaccine series (3 doses) is required for students born on
or after 1/1/93. Children born on or after 1/1/98 are required to have
the varicella (chickenpox) vaccine or a physician’s written proof of
having had the disease. Entrants to grades 6 and 7 are also required to
show immunity to varicella (chicken pox) with written documentation from
their physician of either having the disease or receiving the
vaccination.
Annual Physical Examinations are
mandated for students entering grades K, 2, 4, 7 and 10. Parents are
urged to have their family physician administer such physicals at these
levels. If a physical is not done by the family physician, the school
physician is available to conduct an examination here at school.
School Health Screening: All students are screened for weight, scoliosis, height, vision and hearing each year.
Athletic Program: Students
are required to have a current physical (within 12 months) on file in
order to participate in interscholastic sports. Parents also need to
complete the Medical Recertification/Parent Permission form prior to
each season, to determine the physical capability of a student to
participate. A student needs one physical for all sports, unless an
injury or an extended absence occurs during the year. If a student is
absent from school for five or more consecutive days or receives an
injury, they must obtain clearance from their own physician and approval
from the school physician before returning to competition. All
paperwork needs to be filed with the school nurse before the start of
the first practice.
Medication: The
State Directive regarding medication of any kind (any prescription
medication as well as over-the-counter medications such as cough drops,
cough medicine, aspirin, etc.) states: "If the parent submits a written
request to the school authorities and it is accompanied by a written
request from the family physician in which the frequency and dosage of a
prescribed medication are indicated, then the school nurse may
administer this medication during school hours."
